Thomas H. Cook, of Levittown, died on Nov. 30, 2003. A WWII veteran, he worked for UPS for 32 years and the Island Trees School District for 25 years. Husband of the late Teresa J. Father of John P. and Carol Cook and Matthew M. and Lorrie Cook. Grandfather of Cristi, Thomas, Robert, Carolyn and Lauren Cook. Brother of Elizabeth Wanamaker. Arrangements were made by the Fredrick J. Chapey E. Islip Funeral Home. Interment Pinelawn Memorial Park.
