All present eighth through eleventh graders who plan to play a sport in the fall 2007 must have a doctor's sports physical.
If you did not attend June physicals, you must go to your own physician and you must use the Garden City Public Schools Physical Examination report. The examination report must include: numerical values for height, weight, blood pressure, heart rate, cardiovascular, rhythm, urine for glucose and protein must also be completed. (If this is not included, your child will not be eligible; a full examination, healthcare provider's signature, address, phone number and stamp.
Mail the completed form to the high school nurse by Aug. 8 for the fall season. Do not mail the green form. All physical forms received after Aug. 8 will not be processed in time for the beginning of the fall season.
Additionally, a green health/history permission form must be completed and signed by a parent no more than 30 days prior to the start of each sport. The green form must be completed for each and every sport a student plays. Students will bring the signed green form to the high school café in August. Please be aware these are New York State guidelines, and a student will not be allowed to tryout and/or participate without both the sport physical and the green form.
Monday, Aug, 20:
JV and varsity football begins at 7 a.m. Aug. 20. All boys must be in the high school gym at 7 a.m., with their green form dated in August and plan to be there all day and all week for practice/tryouts (including Saturdays).
All other sports teams begin Monday, Aug. 27. Go to the high school Cafe with your green health/history permission form at the following times:
Monday, Aug. 27:
8 a.m. - all ninth grade boys
8:15 a.m. - all ninth grade girls
8:30-8:45 a.m. - all tenth grade boys and all tenth grade girls
9 a.m. - all eleventh grade boys and all eleventh grade girls
9:15 a.m. - all senior boys and girls
Students bring green form (dated in August) to high school Cafe at time indicated for their grade. After going through the long lines to have the green form checked by our nurses, students will wait for each sports meeting and tryouts to begin, approximately 11 a.m.
All students must attend all tryout and practice sessions all week (most teams have two sessions each day). All teams practice six days a week, on weekends, and school holidays!
