If you plan to play a fall sport at Garden City High School and you did not get a physical from the school doctor on June 10-11, you must go to your own physician.

Take a Garden City white physical examination report form to your private physician. Additional forms are available at the high school. Bring the completed physical examination form to the high school's main office by Aug. 9 or mail it to Mrs. Wyremski at Garden City High School, 170 Rockaway Avenue, Garden City, NY 11530.

All athletes must bring a green health history/permission form on the first day of tryouts. This must be filled out by a parent, signed and dated in August.

Monday, Aug. 23

-7 a.m. football athletes (practice after and all week)

-9 a.m. all other fall athletes (no practice)

Monday, Aug. 30

-8 a.m. all other fall sports (practice after and all week)
